You can take Ekuasi back off and he would be allowed to come back on as a sub. You can do that with non tactical subs. He came on as a HIA sub so could technically come back off and then on

It may have been an accident, but surely if you hit someone in the head at a breakdown it should be a penalty.

If you lay a high tackle accidentally it is still a penalty.
