In regards to the England incidents it's not the first time. In 2003 they had 16 players on the field against Manu Samoa and should have been stripped of their points but were just fined 10,000 pounds.

ENGLAND'S World Cup prospects were still on track last night after they escaped with a pounds 10,000 fine for briefly fielding 16 players against Samoa.
The disciplinary hearing could have taken away the five points earned from the victory - which would have given Clive Woodward's side a tough quarter-final against New Zealand rather than Wales.
England admitted ignoring the instructions of an official and sending Dan Luger onto the field as Mike Tindall received treatment during the final minute of the match.
England's fitness coach Dave Reddin was also given a two-match touchline ban for his part in the incident.
