Oztam base their ratings on a survey of 2,120 households with Foxtel. I'd imagine they won't include a household without Foxtel (on TV) in their sample. The ratings are based on audio - so if anyone in the 2,120 households does use Kayo or similar it would be picked up in the ratings.
